NDIS Social Work
I support clients from a person in environment approach, working holistically, acknowledging how all areas of someone’s life are linked and impact one another. My practise foundation is empowerment which recognises strength and resiliency, walking alongside someone in their journey.
I utilise these approaches to provide support that encompasses Engagement, Identifying barriers, Planning/ implementation and Conclusion.
Support can include:
- Report writing
- Support letters
- Advocacy and referrals
- Narrative therapy within consults
- Intermittent NDIS support coordination if funding is exhausted
Each client’s journey is different and support will be tailored to each individual, supporting their goals.
Specialist Support Coordination
I provide support coordination for plan managed and self-managed clients for Level 2 and 3.
My areas of interest are, but not limited to:
Autism, ADHD, Acquired Brain injuries, clients with co-morbidities.
Support coordination encompasses the following support to a NDIS participant:
- Understand the Plan;
- Connect with Supports and Services;
- Design Support Approaches;
- Establish Supports;
- Coach, Refine, Reflect;
- Targeted Support Coordination;
- Crisis: Planning, Prevention, Mitigation and Action;
- Address Complex Barriers;
- Design Complex Service Plan;
- Build Capacity and Resilience; and
- Report to the NDIA.
